Privacy Statement
The information collected will be used by the school for enrollment and forms an essential part of the information held by the school on your child. The records made from this information may be viewed on request at the school. The information collected may be disclosed to appropriate education, health and welfare authorities and for data-gathering purposes by the New Zealand Ministry of Education, in accordance with the principles of the Privacy Act. It will not be disclosed to any other person or agency unless such disclosure is authorised or required by law.
Parent Declaration
I/We hereby make an application to enrol my son/daughter at Mangere Bridge School under the terms and conditions of the Enrolment which I/we accept as applying to him/her. I/We agree that he/she will be subject to school rules and I/we will endeavour to see that he/she obeys them. I/We understand the school will take action on my behalf in case of injury or sudden illness.
Statement

School manages all personal information it collects from students in accordance with the provisions of the Education and Training Act 2020 and the Privacy Act 2020.

Collection and use of Personal Information - The School collects the information on this enrolment form for the following purposes: 1. enrol your child at school; 2. assess the educational needs of your child; 3. ensure the school get the correct resources from the Ministry of Education for your child.

Access and Storage - Only authorised school staff will have access to your child’s information, and your child’s information may only be accessed for appropriate purposes. Your child’s information is securely stored in the School’s student management system (SMS). Schools without a SMS will complete their roll returns manually using an excel template, which is supplied to the Ministry over a secure network. This information is stored securely and appropriately. You can ask to see the information the schools hold on your child, and request corrections to the information, at any time, by contacting the School.

Disclosure of Personal Information - The School will not disclose or share personal information collected on this enrolment form to third parties unless it is authorised or required to do so under law.

Information Sharing Agreements with other Agencies – Ministry of Education - The Education and Training Act allows the Ministry of Education (the Ministry) to collect personal information about school students without consent from the student (or parent/caregiver). The school will collect the information about your child on this enrolment form and enter it into the school’s SMS, or directly into ENROL. ➢ The schools will send some of this information to the Ministry in the School Roll Return collection. The Ministry will use this information to fund and staff schools, support policy analysis/development, project rolls for the appropriation of Government funding over a 3-year cycle, to monitor the outcomes of the New Zealand education system, undertake research and publish education statistics nationally and internationally.

Ministry of Social Development - Under an Information Sharing Agreement the Ministry shares specified personal information with the Ministry of Social Development (MSD) to support the Youth Service Initiative. The Youth Service identifies young persons who may have difficulty finding future employment, training or further education. The Ministry provides MSD with address and phone number information. The Youth Service uses this information to contact the individuals and support them into employment, training or further education.

Ministry of Health - Under an Information Sharing Agreement, the Ministry shares specified personal information with the Ministry of Health to support the B4 School Checks initiative. The B4 School Check is a nationwide programme offering a free health and development check for 4-year-olds. The Ministry provides Health with information about 5-year-olds enrolled in school to assist in identifying students who have not been offered or completed the B4 School Check.

New Zealand Qualifications Authority (NZQA) - The Ministry share personal demographic information with NZQA to calculate statistics for the Enrolment-Based Measure (EBM). EBM enables detailed analysis and reporting of NCEA attainment, including the production of Secondary Statistics

Statistics NZ - Under an Information Sharing Agreement, the Ministry shares specified personal information with Statistics NZ for use in the Integrated Data Infrastructure (IDI). The IDI is used by authorised researchers for research and statistical purposes. Outcomes of research and statistical analysis using the IDI will not identify any individuals.

Te Whatu Ora - Under an Information Sharing Agreement, the Ministry shares specified personal information with Te Whatu Ora to assist the mitigation of an infectious disease outbreak. The Ministry provides Te Whatu Ora with address and personal data to support ongoing analysis and outbreak management in the case of a notified infectious disease outbreak.
